The completed form can be emailed to our general mail; faxed to 262-375-7656 or mailed or dropped off at Cedarburg Light & Water Utility N30W5926 Lincoln Blvd. Cedarburg, WI 53012.

Email, fax or send along with the application a copy of the certified plat of survey. Provide a drawing showing the proposed location of the service point with dimensions from the nearest corner for approval.

Once CL&W receives all documents and the electric inspection for the service, installation of the electric service lateral will be scheduled.

CL&W or subcontractor are not responsible for damages to private facilities that are not properly marked.

Multiple meter services shall be properly identified with an address or unit number. Appropriate marking shall be permanent adhesive rigid plastic label or individual self-stick letters or numbers with a minimum of ½” block text.

The meter socket shall be installed so the center of the meter will have a maximum height of 5’ and a minimum of 3’-6” above finished grade.

Standard size services under standard digging conditions (no rock, frost, hard surfaces or soil vacuum equipment required due to existing utilities) will be installed at no charge. The standard route is a direct line from the meter pedestal to the nearest point of service distribution. Any requests to modify the proposed utility route will have costs associated with it.

Grades need to be within 6” of final grade.

No temporary service is available for residential customers.

For service relocations contact the utility at 262-375-7650. Provide a drawing showing relocated service point. A site visit may be needed. An estimate will be provided by the utility.
